You may download a registration form in either English or Spanish. Complete and mail the form to the election authority for the jurisdiction in which you live (i.e. if you live in Springfield, the form would be mailed to the Sangamon County Clerk).

You must send with this application either (i) a copy of a current and valid photo identification, or (ii) a copy of a current utility bill, bank statement, government check, paycheck or other government document that shows your name and address.

You may also mail the form to the State Board of Elections, Voter Registration Department at 1020 South Spring, Springfield, IL 62704. The Board's staff will forward the form to the appropriate jurisdiction. The election authority will issue a voter identification card to you after the form is processed.
