The contributor of any in-kind contribution valued in excess of $50 must notify the recipient of the in-kind within 5 business days on an In-Kind Contribution Notification Form.

The notification/certification must include the name and address of the contributor, the name and address of the vendor, the ascertainable/certified market value, and a description of the goods or services, and the date of the expenditure.

This form is never filed with the Board; it simply places responsibility on the contributor to give the recipient information in a timely manner, so that it can be properly reported.
